Prescribing link worker for the Sittingbourne PCN.

My names Paula, I'm a Social Prescribing Link worker for the Sittingbourne PCN.

The role is very varied, we support patients that have things going on alongside their health conditions, we are not clinicians.

These issues might be housing concerns, financial hardship , information and help with social care. We can help to explore what social activities are available locally to help ease social isolation. We are able to signpost and refer you to local services for specialist support.

If you think we may be able to help, please ask for a referral at reception, This will initially be a telephone call.
